Abstract

While the clinical syndromes of COVID-19 infection vary widely between patients, ranging from middle and upper respiratory tract infection to critical multiple organ failure, acute kidney injury (AKI) is a common complication of COVID-19 in up to 46% of patients.[1,2] AKI is associated with higher mortality in patients with COVID-19, [3] but data on renal function after discharge in a large cohort were not available, and a small study observed decreased renal function in 182 COVID-19 patients with AKI.[4] Since AKI is a clinically significant risk factor for chronic kidney disease (CKD), the long-term post-acute consequences of AKI associated with COVID-19 require further investigation. However, to date, there are several key limitations in research on the results of restoring kidney function in AKI associated with COVID-19. First, studies of renal function recovery have lacked adequate serum creatinine levels prior to hospitalization to quantify the decline in renal function after AKI [5,6], and they may underestimate pre-existing undiagnosed CKD prior to hospitalization for COVID-19. Second, renal function outcomes in AKI associated with COVID-19 were determined by either: a) post-discharge renal function assessments, including estimated glomerular filtration rate (eGFR), which significantly underestimated baseline serum creatinine levels4, or b) clinical indicators of serious renal adverse events, including renal replacement therapy (RRT), which underestimate the onset of CKD after the initiating AKI episode. 7 Finally, follow-up periods These cohort studies ranged from 3 to 10 months and did not reflect long-term renal impairment. more than 1 year after AKI[4-8].
